Required forms for all students are due to your third period class the first day of school

Emergency Care Information

All students must have accurate emergency care information.  Parents can update this information through WeCare@School. Only update in WeCare if there are changes. A completed signed emergency care form should also be returned on the first day of school to their third period teacher.

Student's Rights and Responsibilities

Review the Student's Rights and Responsibilities with your child.

 

Bring the signed  Parent and/or Guardian Signature Sheet on the first day of school to their third period teacher.

Health Information Form Complete the Health Information Form and turn in the first day of school to their third period teacher.

Military Connected Student Form

Form only needs to be returned if there is a change in parent's military connected status.

High School Science Safety Agreement

Any student enrolled in a science course should print the High School Science Safety Agreement form and bring it signed on the first day of school. If the student has more than one science course they will need to bring a signed copy to each science teacher.

Bus Transportation

Bus routes are not mailed, specific information regarding bus route and stop will be available in your child's SIS account.

Parents can use this online payment system to check balances and add money to school cafeteria accounts, to make payments to the school for various school related items and to receive low account balance notifications.

Medication Drop Off

The Health Room will be open for drop-off of emergency and daily medications on August 22, 9:00 a.m. - 11:00 a.m. All medications are required to have a medication authorization form.

Parents or guardians must transport medications to the school. This includes prescription medication. With parent or guardian approval, a high school student may carry an unopened over-the-counter (OTC) medication directly to the school health room. All medications must be in the original container accompanied by the appropriate Medication Authorization listed above.
